Transportation officials released results of their investigation into a helicopter crash in northern Alberta that left two dead and another in hospital in the spring of 2013.

Back on May 29, 2013, a helicopter with three people on board was flying north of Fort McMurray when the aircraft crashed.

At the time, officials said the pilot, identified as Bryce Campbell from Golden, B.C., and biologist Kristina Norstrom died at the scene, the third individual identified as biologist Simon Slater was rushed to hospital with serious injuries.

Officials with the Transportation Safety Board of Canada (TSB) said Wednesday that the effectiveness of the tail rotor was lost mid-flight, which investigators said caused the chopper to lose altitude, and made it impossible for the aircraft to recover before the crash.

According to TSB officials, since the crash, Wood Buffalo Helicopters had altered the pilot training forms to show instruction on the loss of tail rotor effectiveness had been completed during ground and flight training programs.
